New Brunswick

Historic New Brunswick vital records (those earlier than the dates below) are indexed on the Provincial Archives of New Brunswick website. Some digital images are also provided.

The Vital Statistics Office under the Department of Health and Wellness has custody of (as of January 2014):

  • Births from 1918
  • Marriages from 1963
  • Deaths from 1963

The certificates may be ordered from Service New Brunswick by mail or online:

A person applying for a birth or marriage certificate who is not the person named on the certificate, must provide written consent of the person named on the certificate, unless the person is a parent applying for a minor child’s certificate. The consent must be dated and signed by the person named on the certificate. This written consent must be faxed or mailed along with the application to Vital Statistics in order for the application to be processed. Certificates will not be issued without this written consent. Obviously, if someone is applying for a deceased person's birth or marriage certificate, a written consent is not possible. Please submit proof of death with your application.

In order to apply for and obtain a certificate, you will need to provide:

A. the following information for:

Birth Certificate
  • Name of child as registered
  • Date of birth
  • Place of birth
  • Names of both parents, maiden name of mother, places of birth
Marriage Certificate
  • Name of bride at time of marriage and place of birth
  • Name of bridegroom at time of marriage and place of birth
  • Date of marriage
  • Place of marriage
Death Certificate
  • Name of deceased
  • Date of death
  • Place of death
  • If known, funeral home, spouse’s name, if married

B. Your reason for the request

C. Relationship to the person named on the certificate

D. Consent, if applicable

E. Type of certificate requested

F. A cheque or money order made payable to the Minister of Finance in the appropriate amount or by providing your Visa or MasterCard number with expiration date

G. Full return mailing address and daytime telephone number

What information is included on the certificates?

There are two types of certificates: Wallet size or Long Form. Wallet size Birth Certificate

  • Name of Individual
  • Date of birth
  • Place of Birth
  • Sex
  • Registration date
  • Registration number
  • Date issued

Long Form

All of the above information is included plus the names and the province or country of birth of the parents

Wallet size Marriage Certificate

  • Name of the bride at the time of the marriage
  • Name of the bridegroom at the time of the marriage
  • Date of the marriage
  • Place of the marriage
  • Registration Number
  • Registration date
  • Date issued

Long form includes all of the above plus

The province or country of birth of the bride and bridegroom.
A photographic print of a marriage registration is available with the written consent of one of the parties to the marriage.

Long Form copy of Death Certificate

  • Name of the deceased
  • Date of death
  • Place of death
  • Date of birth
  • Province or country of birth of the deceased
  • Registration number
  • Registration date
  • Date issued
