Notes

An index for Illinois, Cook County, Birth Certificates is available online, click here.

Microfilm of original records at the Cook County Courthouse, Chicago, Illinois.

Many sequences of numbers missing within film number ranges in the film notes.

In at least one instance, a certificate number found in the index is found twice for the same year (1895) in the sequences of certificate numbers in the inventory below. Check through the sequences of numbers for the year of your subject's birth to verify whether or not the subject's number is found more than once for that year. If the number is found more than once, you may be able to guess which roll of film to choose because there is some degree of chronological order to the certificates.

The title of the index to these records is, "Cook Co., Ill. Birth Index, 1871-1916." (See "References" on this page.)
