Only a few of the "Affidavit and Certificate of Correction" certificates are indexed in "Cook Co., Ill., Birth Index, 1871-1916," microfiche #6016532 in 83 parts.

There are "Certificate of birth" documents that are actual birth records & are intermixed with the birth correction certificates. Most of these documents are not indexed.

The numbers associated with these records are certificate numbers.
