Q. Can the parents’ surname be assumed from the child’s surname on a Death Certificate?  A. No. The Field Helps for the Father’s and Mother’s surname state: If the surname was not recorded or was written as a variation of the word 'unknown,' press Tab to skip this field.

Q. How can I tell the difference between the record and page numbers?  A. Look at these examples of a page number and a record number.  The numbers may be stamped or handwritten. Refer to the field help for where to find the number.
