Not just for this page, but for all the pages you create which involve Catholic records, the proper citation would descend from the universal ("Catholic Church") to the particular diocesan church ("Diocese of _____") to the parish level ("Parish of _______") and thereafter to day chapels, oratories, chapels at ease, religious institutes, or votive chapels. In order to document the records series properly, it is necessary to identify the diocese. "Our Lady of Comfort" (rough translation) in two cities is clearly the parish level. I recognize that the differences between civil and canonical jurisdictions may be new to you.
